Dear Active and Retired Members of the Ohio Troopers Coalition,

 

As an active Dues Paying OSTA Member or OSTRA Member, you are entitled to a blanket Four Thousand Dollar ($4,000.00) Accidental Death and Dismemberment Benefit through American Income Life.  Regardless if it is on the job or not, as long as it is declared an accident, the member is covered.  If the member should pass due to (including but not limited to) disease, natural causes, suicide, being under the influence of drugs or alcohol, the benefit would not pay out.  *(See Note – bullet 2)

 

As an active Dues Paying Member of the Ohio State Troopers Association Bargaining Units 1 or 15, you are entitled to a blanket Twenty-Five Hundred Dollar ($2,500.00) Death Benefit through Ohio Troopers Coalition.  Regardless if it is on the job or not, if the member should pass due to (including but not limited to) disease or natural causes, the benefit would pay out.  Any loss falling within the list of Policy Exclusions (including but not limited to) suicide, member being under the influence of alcohol or the voluntary taking of drugs not prescribed by a doctor or committing or attempting to commit a felony or assault will NOT be eligible for benefits.

 

As a dues paying retired member you are also entitled to a blanket Two Thousand Dollar ($2,000.00) Death Benefit through the Ohio State Trooper Retirees Association (OSTRA) regardless of your age at the time of death.  Please note that if the member should pass from suicide, being under the influence of alcohol or the voluntary taking of drugs not prescribed by a doctor or committing or attempting to commit a felony or assault, the benefit will not pay out. 

 

These benefits are provided by the Ohio Troopers Coalition (OTC) and/or the Ohio State Trooper Retirees Association (OSTRA) at no additional cost to the member. Benefit limits for the OTC or OSTRA Death Benefits (age requirements and dollar amounts) are subject to change per the Combined Board. 

 

To avoid needless delay in initiating payment of benefits to the member’s beneficiary, we request that you designate a primary and contingent beneficiary on the “Beneficiary Designation” form and forward it to the OTC.  The Beneficiary Designation form is available by clicking the link below.

 

We request that you designate one individual as your Primary Beneficiary.  In the event of passing, the primary beneficiary will receive a lump sum payment of the benefit.  You may also designate a contingent beneficiary should your primary beneficiary not survive you then the lump sum will be made to your contingent beneficiary as specified.

 

Any benefit paid to a minor beneficiary shall be made to the legally appointed guardian of the minor, unless otherwise permitted by law. 

 

If you do NOT name a beneficiary, or if no named beneficiary survives you, payment of the benefit will be made to the beneficiary/beneficiaries shown below in the order listed:

  • The spouse to whom you are married when death occurred; or if none
  • A child (see above if a minor) ** Please note that benefit cannot be split amongst multiple surviving beneficiaries.; or if none
  • Your parents or surviving parent;

 

In the event that you would like to change your beneficiary information, please complete a new Beneficiary Designation form and forward to OTC.  The new Beneficiary Form filed will supersede any previous designation.

*Notes:

 

1.       Benefits are only eligible to DUES PAYING MEMBERS of both OSTA and OSTRA.

2.       Any loss falling within the list of Policy Exclusions (including but not limited to disease, suicide, member being under the influence of alcohol or the voluntary taking of drugs not prescribed by a doctor, natural causes or committing or attempting to commit a felony or assault) will NOT be eligible for benefits.

3.       A death certificate must be given within 90 days after the date of the loss.  Failure to provide information (of loss or beneficiary nature) shall result in the failure to pay benefits.

4.       Benefit limits are subject to change per the Combined Board.
